REQUIREMENTS
Passport
  • Has to have at least two blank visa pages.
  • Be valid 6 months after the entry date into Ukraine.
A clear photocopy of the applicants passport Bio info page(s), this includes the photo.

Photographs
  • Two recent passport sized color photographs.
Proof of Residency:
  • UK visa or residency permit (if applicable), has to be original and valid at least 6 months on the submission day.
  • Also supply a photocopy.
  • Not needed if the applicant is applying on an EU passport, otherwise see below.
A certified copy of the work permit issued by Ukraine's State Centre for Employment
Proof of Medical Insurance covering at least 45 days from the date of entry into the Ukraine.
Proof of sufficient funds to cover stay in Ukraine (e.g. Bank statement) or a letter from the employer confirming their coverage of applicant's expenses in Ukraine.
